DOT SAP Evaluation in Ohio — 14 Providers Listed
Ohio is one of the country's biggest distribution states: I-70 and I-71 through Columbus, I-75 through Toledo, Dayton and Cincinnati, the Turnpike across the north through Cleveland, and the Rickenbacker inland port south of Columbus. Auto parts, steel and consumer goods move through the state every direction. The SAP process itself is federal — 49 CFR Part 40, Subpart O — so nothing about it changes at the Ohio line. What Ohio controls is the license: the Ohio Bureau of Motor Vehicles downgrades the CDL of a driver in prohibited status, as every state has been required to do since November 18, 2024 (FMCSA), and reinstates it once the Clearinghouse shows “not prohibited.”
A remote evaluation is accepted in Ohio because the federal rule permits it everywhere: a SAP may evaluate you by real-time video at their discretion (49 CFR §40.291), provided their own license lets them see a driver in Ohio. A SAP across the line in Michigan, Pennsylvania, West Virginia, Kentucky and Indiana is also an option, in person or by video, under the same licensing question. The return-to-duty test cannot be remote: it is an observed collection, ordered by an employer or C/TPA, so plan for one trip to a collection site.

Losing the license and getting it back are two different processes run by two different bodies. The prohibition is federal — it lives in the FMCSA Clearinghouse and only moves when your return-to-duty steps are reported there. The license itself is Ohio’s, and Ohio Bureau of Motor Vehicles acts on what the Clearinghouse tells it. Neither one can be talked into moving early, which is why the only thing that actually shortens this is starting the evaluation sooner.

- “Within ten calendar days after receiving information from the federal motor carrier safety administration that a commercial driver’s license holder is no longer prohibited from operating a commercial motor vehicle, the registrar must initiate reinstatement procedures.” source

Ohio is the most specific state here about the back half of the process: the registrar has ten calendar days from the FMCSA notification to start putting your privilege back. Once your return-to-duty test is reported and the Clearinghouse reads Not Prohibited, that is a number you can hold Ohio to. What the BMV cannot do is change your Clearinghouse status or lift the downgrade on its own — that only moves when FMCSA says it moves.
The order it has to happen in
- Get evaluated by a DOT-qualified SAP. This is the only step you can start today, and every other date on this page counts from it. The SAP must meet the credential rules in 49 CFR 40.281 — a regular counselor or your family doctor cannot do it.
- Finish the education or treatment the SAP prescribes. The SAP decides what and how much; nobody else can shorten it, and a second opinion from a different SAP is not permitted (§40.293).
- Pass the return-to-duty test, observed, arranged through an employer or C/TPA rather than booked by you (§40.305).
- The negative result is reported to the Clearinghouse and your status changes to “not prohibited.” This is the moment Ohio is waiting for — nothing you file with the state before this point moves anything.
